You'll work closely with a supervising Physical Therapist to carry out individualized treatment plans that foster mobility, strength, balance, and independence. Responsibilities include:

• Implementing prescribed physical therapy treatments, including therapeutic exercises, transfers, and gait training
• Documenting patient responses, reporting changes, and contributing to care planning using Home Care Home Base (HCHB)
• Educating patients and caregivers on exercises, safety, assistive device use, and fall prevention strategies
• Traveling daily to visit patients in their homes throughout your assigned territory
• Collaborating with an interdisciplinary team and participating in case conferences
• Ensuring compliance with agency policies and regulatory guidelines

Qualifications:

• Active and unrestricted Physical Therapist Assistant (PTA) license in state of practice
• At least 1 year of recent home health PTA experience (field work preferred)
• Proficiency with electronic documentation systems, preferably HCHB; familiarity with OASIS processes is a plus
• Strong clinical decision-making, observation, time management, and communication skills
• Reliable transportation, valid drivers license, and active auto insurance
• Self-starter able to independently manage a full, flexible schedule
