You'll work closely with a supervising Physical Therapist to carry out individualized treatment plans that foster mobility, strength, balance, and independence. This full-time, 13-week contract is ideal for a self-motivated, compassionate professional eager to make a real difference while thriving in a field-based, autonomous environment.
Responsibilities include:
- Implementing prescribed physical therapy treatments, including therapeutic exercises, transfers, and gait training
- Documenting patient responses, reporting changes, and contributing to care planning using Home Care Home Base (HCHB)
- Educating patients and caregivers on exercises, safety, assistive device use, and fall prevention strategies
- Traveling daily to visit patients in their homes throughout your assigned territory
- Collaborating with an interdisciplinary team and participating in case conferences
- Ensuring compliance with agency policies and regulatory guidelines
Qualifications:
- Active and unrestricted Physical Therapist Assistant (PTA) license in state of practice
- At least 1 year of recent home health PTA experience (field work preferred)
- Proficiency with electronic documentation systems, preferably HCHB; familiarity with OASIS processes is a plus
- Strong clinical decision-making, observation, time management, and communication skills
- Reliable transportation, valid drivers license, and active auto insurance
- Self-starter able to independently manage a full, flexible schedule
