A skilled Physical Therapist with home health expertise is needed for a contract position in the Tampa, FL area. This role involves delivering specialized therapy services to patients with cardiopulmonary, neurological, orthopedic, geriatric, and oncology needs. The position requires a minimum of two years of experience, proficiency in assessment and documentation, and thorough knowledge of equipment and devices used in home health therapy.

Responsibilities:
- Conduct home skilled visits, completing 6 to 8 visits daily (minimum 6 points per day).
- Perform patient assessments and develop home exercise programs tailored to individual needs.
- Utilize equipment such as crutches, gait belts, walkers, sliding boards, and supplemental oxygen delivery systems.
- Educate patients and their families on medications, safety, injury prevention, and identification of abuse signs and symptoms.
- Document all care accurately using electronic systems including BayCare Forms, BayCare Scheduling, and Ace Timesheet.
- Maintain compliance with COVID-19 vaccination or exemption policies and flu shot requirements, adhering to safety protocols including mandatory mask-wearing.
- Self-schedule visits and hours based on office and patient needs.
Qualifications & Desired Experience:
- Valid physical therapy license and BLS certification.
- At least 2 years of home health experience; OASIS training preferred.
- Proficient in cardiopulmonary, neuro, ortho, geriatrics, and oncology patient care.
- Ability to drive and reliable transportation for home visits.
- Comfortable working with varying equipment and devices used in home health settings.
Schedule:
- Monday through Friday, Day Shift (8:00 AM 5:00 PM)
- Every third weekend required.
