Job Description

Provide expert physical therapy care in a home health setting, delivering evaluations and treatment plans that promote patient independence and functional improvement. This contract opportunity is based in Ronceverte, WV, covering Pocahontas County, with a weekday schedule from 8 a.m. to 5 p.m., no weekends or on-call duties.

Key Responsibilities:

  • Conduct comprehensive physical therapy evaluations in accordance with physician orders and patient history.
  • Develop and implement individualized treatment plans with measurable goals.
  • Monitor and adjust treatment based on patient progress and clinical judgment.
  • Communicate effectively with patients, families, and healthcare teams regarding care and progress.
  • Recommend adaptive equipment to enhance rehab outcomes.
  • Maintain accurate documentation and ensure compliance with company policies and legal regulations.
  • Supervise Physical Therapist Assistants and Aides, providing guidance in patient care.
  • Participate in continuing education and staff development activities.

Qualifications and Desired Experience:

  • Valid West Virginia PT license or Compact license accepted.
  • Familiarity with homecare, OASIS documentation, exercise programming, and rehab principles.
  • Experience with Home and Community-based Health Block Grants (HCHB) is a strong plus.
  • Strong communication and organizational skills.
  • Ability to work independently and manage time effectively.
