Job Description
Deliver compassionate care and support to adults in a home health setting, working with a diverse population managing chronic conditions and post-operative needs. Therapists play a key part in helping patients regain independence and mobility through evidence-based interventions and education, all within a collaborative team culture focused on patient outcomes.
Schedule/Hours:
- 6 patient visits per day (average)
- Mileage travel averages 60-80 miles per day across Albemarle, Badin, Harrisburg, Mt Gilead, Norwood, Troy, and nearby areas
- Flexible schedule with no weekend rotation, no call, and no major holidays required
- Schedule cycle is every 13 weeks, RTO requires manager approval
Required Qualifications:
- Minimum of 1 year of home health experience required (seasoned home health travelers accepted)
- Valid Drivers License and proof of insurance
- Active state PT license and current BLS certification required
- Strong clinical skills in chronic diseases, orthopedics, and/or rehabilitation
- Ability to independently schedule patient visits and manage travel
- Comfort in a technology-forward environment (using EPIC Rover, Focura, Samsung Note 9, and Home Care Home Base)
- Reliable personal transportation
Key Responsibilities:
- Deliver high-quality therapy interventions in patients homes
- Support patients across a range of diagnoses, focusing on maximized functionality and quality of life
- Coordinate care and scheduling with patients directly
- Maintain thorough, timely documentation
- Collaborate closely with department leadership, the scheduler, clinical manager, supervisor, and peers
