Job Description

A healthcare role is available for an experienced Physical Therapist ready to bring expert care and rehabilitation services to patients in various county settings. This contract position offers a structured Monday through Friday schedule, 8 am to 5 pm, without weekend or on-call duties, providing balance and predictability in your workweek.

The ideal candidate holds a current NC PT License or a compact license recognized by North Carolina and demonstrates proficiency in conducting thorough physical therapy evaluations and treatments based on physician referrals. Experience or familiarity with HCHB software is a plus, enhancing your ability to document and manage patient care effectively.

Key Responsibilities:

  • Conduct comprehensive physical therapy evaluations using standardized assessment tools.
  • Develop individualized treatment plans with measurable goals aimed at maximizing patient independence and function.
  • Reassess patient progress regularly and adjust treatment plans as needed.
  • Communicate effectively with patients, families, and healthcare teams regarding treatment plans and progress.
  • Collaborate with medical providers on patient care and discharge planning.
  • Recommend adaptive equipment to improve patient outcomes and incorporate it into rehabilitation programs.
  • Maintain accurate and timely documentation compliant with company policies and regulatory requirements.
  • Supervise Physical Therapist Assistants and Aides in patient care activities following state practice acts.
  • Participate in ongoing professional development and internal training programs.
