Key Qualifications and Experience:
- Valid WV physical therapy license or a compact license accepted
- Experience in homecare, OASIS documentation, exercise programming, and rehabilitation preferred
- Familiarity with HCHB systems is a plus
- Ability to conduct comprehensive PT evaluations and develop measurable treatment goals
- Strong communication skills to collaborate with medical providers, patients, and families
- Capability to supervise Physical Therapist Assistants and Aides under state practice act guidelines
Primary Responsibilities:
- Evaluate and treat patients based on physician referrals and orders, utilizing appropriate assessment tools
- Monitor patient progress and update treatment plans to achieve functional independence goals
- Consult with healthcare team members and recommend adaptive equipment as needed
- Complete all required documentation accurately and within specified timelines
- Adhere strictly to company conduct policies, HIPAA, and regulatory standards
- Participate in ongoing staff education to maintain professional competencies
