A home health physical therapy role is available in Dunedin, FL, offering a contract opportunity focused on providing specialized care in patients residences. This position is ideal for a licensed PT with experience in home health settings who values autonomy and the chance to work closely with a diverse patient population.

Responsibilities:
- Conduct home skilled visits averaging 6 to 8 patients per day, focusing on cardiopulmonary, geriatrics, neuro, ortho, and oncology specialties
- Perform thorough assessments and develop home exercise programs tailored to individual patient needs
- Manage documentation using electronic systems for scheduling and record-keeping
- Educate patients and their families on medications, safety, and injury prevention
- Utilize a range of equipment and devices such as crutches, gait belts, hospital beds, and supplemental oxygen delivery systems
- Adhere to infection control protocols, including mandatory mask wearing and vaccination requirements
Qualifications and Desired Experience:
- Licensed Physical Therapist with a minimum of 2 years home health experience
- Knowledge of OASIS documentation preferred
- Current BLS certification
- Valid drivers license and reliable transportation for home visits
- Proficient in patient assessment and skilled in managing cardiopulmonary and orthopedic conditions
- Ability to self-schedule visits based on office needs including every third weekend
Schedule:
- Day shift, 5 days per week, 8-hour shifts from 8:00 AM to 5:00 PM
