Job Description

Provide skilled physical therapy services in a home health setting, delivering specialized care to diverse patient populations including cardiopulmonary, geriatrics, neuro, ortho, and oncology. This role involves conducting thorough patient assessments, developing and implementing individualized home exercise programs, and educating patients and their families on medications, safety, and injury prevention. Candidates will deliver care using a variety of equipment such as crutches, gait belts, hospital beds, and supplemental oxygen delivery systems.

Responsibilities include managing 6-8 skilled home visits daily, documenting accurately using digital systems, and attending to patients with sensitivity to physical, sexual, and psychological abuse signs. The position requires flexibility with self-scheduled hours and every third weekend coverage. Careful adherence to infection control protocols including COVID vaccination or exemption documentation, flu vaccination or mask-wearing, are mandatory.

Qualifications and Desired Experience:

  • Minimum 2 years of home health physical therapy experience
  • State licensure and Basic Life Support (BLS) certification required
  • Proficient in assessment, cardiopulmonary and ortho/neuro rehab techniques
  • Experience with OASIS data collection or similar preferred
  • Valid drivers license and reliable vehicle for in-home visits
  • Strong documentation skills with ability to use electronic forms and timesheets

Schedule & Hours:

  • Day shift, 5 days a week, 8-hour shifts (8:00 a.m. 5:00 p.m.)
  • Every third weekend required
