Key Responsibilities:
- Conduct thorough physical therapy evaluations using appropriate assessment tools.
- Develop and implement individualized treatment plans based on physician referrals and patient needs.
- Monitor patient progress regularly, adjusting treatment goals as necessary.
- Collaborate with healthcare providers, family members, and the treatment team to ensure coordinated care.
- Recommend and facilitate the use of adaptive equipment to enhance patient rehabilitation.
- Maintain accurate documentation in compliance with regulatory standards.
- Supervise Physical Therapist Assistants and Aides in patient care activities.
- Participate in ongoing professional development and staff meetings.
Qualifications & Desired Experience:
- Current WV Physical Therapist license (Compact license accepted)
- Experience in homecare physical therapy with familiarity in OASIS documentation
- Knowledge of exercise programming and rehabilitation protocols
- Preferred: Experience with Home and Community-Based Services (HCHB)
- Ability to communicate effectively with patients and multidisciplinary teams
Schedule & Details:
- Hours: 8 AM to 5 PM, Monday through Friday
- No weekends or on-call responsibilities
- Mileage reimbursement available for travel within Pocahontas County
