Job Description
As a physical therapist working in a hospital-based home health setting, you will deliver high-quality rehabilitative care to adult patients with a variety of chronic diseases and post-orthopedic surgery needs. This contract position offers autonomy in scheduling while ensuring extensive support from experienced departmental leadership, schedulers, and your clinical team.
Schedule:
- No required weekend rotation
- No call obligations
- No coverage on major holidays
- 30 visit equivalents per week required
- 6 patient visits per day on average
Key Qualifications:
- Active state licensure as a Physical Therapist (PT) is required
- Current BLS certification
- Valid drivers license and proof of auto insurance
- At least 1 year of home health experience (will consider seasoned home health professionals as first-time travelers)
- Strong clinical assessment and documentation skills
- Ability to travel an average of 60-80 miles per day across designated local areas (mileage reimbursed weekly)
- Proficient in using mobile technology for documentation (EPIC Rover, Focura, Home Care Home Base, Samsung Note 9)
Role Responsibilities:
- Provide in-home therapy sessions to adult patients, assessing needs and formulating individualized treatment plans
- Focus on improving mobility, managing chronic conditions, and aiding recovery after orthopedic surgeries
- Accurately schedule upcoming daily visits directly with your patients
- Document all patient encounters and care plans using specified mobile platforms
- Maintain consistent compliance with clinical and organizational standards
- Collaborate closely with schedulers, managers, supervisors, and peer clinicians in a supportive, team-oriented culture
