Travel RN Case Manager - Rochester, NY (13 weeks)

Sunbelt Staffing is seeking a compassionate Travel RN Case Manager to join our team in Rochester, NY. You'll play a vital role in coordinating care for patients in their homes, ensuring a smooth transition from hospital to recovery.

The Role:

  • Collaborate with patients, families, physicians, and the care team to develop and revise personalized care plans.
  • Conduct comprehensive OASIS assessments and ensure timely documentation.
  • Process orders, notify physicians of patient needs, and complete discharge summaries.
  • Educate patients and families about disease processes, self-care, and preventive strategies.
  • Supervise Licensed Practical Nurses (LPNs) and ensure quality care delivery.
  • Promote patient well-being and advocate for patient rights.


  • Active RN license in New York State.
  • Certified Home Health Care experience required.
  • Proficiency in EPIC or Netsmart systems.
  • Minimum 2 year's of experience in case management or a related field.
  • Strong assessment, communication, and collaboration skills.
  • Ability to work independently and manage multiple priorities.
