Job Description

We are hiring a Care Manager Registered Nurse for a 13-week contract position in Glens Falls, New York. As a vital member of our healthcare team, you will serve as a Care Manager/Case Manager on our Med Surg Tele unit. This contract opportunity offers a Monday through Friday schedule from 8:00 am to 4:30 pm, allowing you to utilize your expertise in care coordination and patient advocacy while maintaining a regular work schedule.

Responsibilities:

  • Serve as a Care Manager/Case Manager on our Med Surg Tele unit, coordinating patient care and ensuring seamless transitions across the healthcare continuum.
  • Conduct comprehensive assessments, develop individualized care plans, and collaborate with the interdisciplinary team to ensure optimal patient outcomes.
  • Advocate for patients' needs, facilitate timely access to appropriate resources and services, and promote patient education and self-management.
  • Collaborate closely with physicians, social workers, and other healthcare professionals to coordinate care plans and discharge plans.
  • Utilize your strong communication and organizational skills to maintain accurate documentation and ensure compliance with regulatory guidelines.

Qualifications:

  • Valid registered nurse (RN) license in the state of New York.
  • Minimum of two years of related experience as a Care Manager or Case Manager.
  • Experience in a Med Surg Tele unit is highly preferred.
  • Proficient knowledge of care coordination, discharge planning, and utilization review processes.
  • Excellent interpersonal skills, critical thinking abilities, and attention to detail.
  • Strong understanding of healthcare regulations and guidelines.
