Job Description

We are hiring a Care Manager Registered Nurse for a 13-week contract position in Glens Falls, New York. As a vital member of our healthcare team, you will serve as a Care Manager/Case Manager on our Med Surg Tele unit. This contract opportunity offers a Monday through Friday schedule from 8:00 am to 4:30 pm, allowing you to utilize your expertise in care coordination and patient advocacy while maintaining a regular work schedule.

Responsibilities:

Serve as a Care Manager/Case Manager on our Med Surg Tele unit, coordinating patient care and ensuring seamless transitions across the healthcare continuum.
Conduct comprehensive assessments, develop individualized care plans, and collaborate with the interdisciplinary team to ensure optimal patient outcomes.
Advocate for patients' needs, facilitate timely access to appropriate resources and services, and promote patient education and self-management.
Collaborate closely with physicians, social workers, and other healthcare professionals to coordinate care plans and discharge plans.
Utilize your strong communication and organizational skills to maintain accurate documentation and ensure compliance with regulatory guidelines.

Qualifications:

Valid registered nurse (RN) license in the state of New York.
Minimum of two years of related experience as a Care Manager or Case Manager.
Experience in a Med Surg Tele unit is highly preferred.
Proficient knowledge of care coordination, discharge planning, and utilization review processes.
Excellent interpersonal skills, critical thinking abilities, and attention to detail.
Strong understanding of healthcare regulations and guidelines.
