Job Opportunity: Public Health Nutritionist

Location: Chicago, IL

Hours: 35 hours/week, Monday to Friday, 8:00 am - 4:00 pm

Position Type: Contracted Position

Description:

Are you passionate about promoting good nutrition and improving health outcomes? We are seeking a dedicated Public Health Nutritionist to join our team in Chicago. In this role, you will interview clients to gather medical history and dietary intake information, identify risk factors, and design personalized care plans to address nutritional needs. You will work closely with medical providers to ensure prescribed care aligns with the nutritional requirements of clients, provide education and counseling on nutrition practices, and develop individualized care plans for clients with chronic diseases. Additionally, you will collaborate with a team of healthcare professionals, document services provided, and conduct nutrition-related health promotion activities.

Projects:

As a Public Health Nutritionist, you will participate in data aggregation and performance improvement activities, ensure timely completion of reports to meet grant deliverables, and provide group nutrition education sessions.

Minimum Qualifications:

  • Master's Degree in Dietetics, Foods, and Nutrition, or a related field
  • Three (3) years of work experience in public health nutrition
  • Registered Dietitian (RD) credential awarded by the Commission on Dietetic Registration
  • Licensed Dietitian Nutritionist (LDN) in the State of Illinois
