An established hospital near Sacramento is seeking a skilled Care Coordinator with a nursing license (RN-MS) for a contract assignment. This role requires a candidate with strong expertise in discharge planning and utilization management to support patient transitions and resource optimization effectively.
Location: Roseville, CA

Responsibilities:
- Coordinate comprehensive discharge planning processes ensuring smooth patient transitions
- Manage utilization reviews to optimize care delivery and resource use
- Collaborate with interdisciplinary teams to support patient-centered care
- Utilize clinical knowledge to assess patient needs and coordinate appropriate services
Qualifications and Experience:
- Active Registered Nurse license (RN-MS) and California state licensure
- Minimum of 2 years hospital experience, preferably in care coordination or related clinical roles
- Current Basic Life Support (BLS) certification
- Proven experience with discharge planning and utilization management
- Familiarity with Interqual criteria is highly preferred
