An established hospital near Sacramento is seeking a skilled Care Coordinator with a nursing license (RN-MS) for a contract assignment. This role requires a candidate with strong expertise in discharge planning and utilization management to support patient transitions and resource optimization effectively.

Responsibilities:

  • Coordinate comprehensive discharge planning processes ensuring smooth patient transitions

  • Utilize clinical knowledge to assess patient needs and coordinate appropriate services

Qualifications and Experience:

  • Active Registered Nurse license (RN-MS) and California state licensure
  • Minimum of 2 years hospital experience, preferably in care coordination or related clinical roles
  • Current Basic Life Support (BLS) certification
  • Proven experience with discharge planning and utilization management
  • Familiarity with Interqual criteria is highly preferred
