A dedicated Registered Nurse (RN) is needed for a full-time home health contract in Zachary, LA. This 13-week assignment offers an excellent opportunity to deliver skilled nursing care in patients homes while enhancing your clinical and documentation expertise. If you have experience with OASIS assessments and managing daily visits in home health, this role allows you to make a meaningful difference in patient outcomes.
Contract Details:
- 13-week duration with potential for extension
- Full-time schedule, Monday through Friday
- On-call duties likely
- Structured daily patient visit expectations
Responsibilities:
- Provide comprehensive in-home nursing care and skilled interventions
- Conduct patient assessments and complete OASIS documentation accurately and timely
- Collaborate with physicians and multidisciplinary care teams to tailor individualized care plans
- Perform wound care, medication management, patient education, and disease monitoring
- Report patient condition updates and changes to the clinical team promptly
Qualifications:
- Active RN license
- At least 1 year of recent home health nursing experience
- Demonstrated proficiency with OASIS documentation (required)
- Experience with HomeCare HomeBase is strongly preferred
- Ability to manage a structured, full-time schedule with reliable transportation and valid drivers license
