We're seeking a skilled Registered Nurse (RN) with proven OASIS experience for a 13-week, full-time home health assignment. Schedule:
- Full-time, Monday through Friday
- Potential for on-call responsibilities
- Structured daily patient visit requirements
Key Responsibilities:
- Deliver high-quality in-home nursing care and skilled interventions to diverse patient populations
- Perform comprehensive patient assessments and complete OASIS documentation precisely and timely
- Collaborate closely with physicians and interdisciplinary care teams to create and implement individualized care plans
- Conduct wound care, medication administration, disease monitoring, and patient/family education
- Maintain regular communication regarding conditions, updates, and care adjustments
Desired Qualifications:
- Active RN license in good standing
- At least one year of recent RN experience in home health
- Required prior experience with OASIS documentation
- Strong preference for proficiency in HomeCare HomeBase software
- Organizational skills and comfort managing a structured case schedule
- Dependable transportation and a valid drivers license
