Your next nursing opportunity starts here. We are seeking a dedicated Registered Nurse with strong home health experience, ready to take on a meaningful 13-week full-time contract. Schedule:
- 13-week contract with potential to extend
- Full-time, Monday through Friday
- On-call responsibility may be required
- Structured daily visit expectations
Key Responsibilities:
- Deliver skilled in-home nursing care and interventions
- Conduct accurate patient assessments and complete OASIS documentation promptly
- Partner with physicians and care teams to develop and implement individualized care plans
- Perform wound care, medication administration, patient education, and chronic disease monitoring
- Communicate patient progress and condition changes effectively to the clinical team
Qualifications:
- Active RN license
- Minimum of 1 year recent home health RN experience
- Proficiency with OASIS documentation required
- HomeCare HomeBase experience strongly preferred
- Reliable transportation and valid drivers license
- Ability to manage a structured full-time schedule and daily field visits
