Job Description

This 13-week full-time contract position is designed for an RN who excels in delivering compassionate in-home care while maintaining precise clinical documentation.

Responsibilities include providing skilled nursing interventions during patient visits, conducting thorough assessments, and completing OASIS documentation accurately and promptly. The role involves collaboration with physicians and care teams to implement individualized care plans, performing wound care, managing medications, educating patients, and monitoring chronic conditions. Effective communication of any changes in patient status to the clinical team is essential.

Qualifications:

  • Active RN license
  • Minimum of one year recent experience in home health nursing
  • Proficiency in OASIS documentation is required
  • Experience with HomeCare HomeBase strongly preferred
  • Ability to manage a structured, full-time schedule with daily patient visits
  • Reliable transportation and valid drivers license

Schedule:

  • Full-time, Monday through Friday
  • On-call responsibilities possible
