We're seeking a Registered Nurse with OASIS expertise to serve patients through a full-time, 13-week home health assignment. This position offers the ideal combination of clinical independence, professional growth, and valuable impact on patient wellbeing outside the hospital setting.
Schedule:
- Full-time, Monday through Friday
- Occasional on-call responsibilities
- Structured daily visit requirements
Key Responsibilities:
- Deliver skilled in-home nursing interventions, including wound care, medication management, and disease monitoring
- Conduct thorough patient assessments and timely, accurate OASIS documentation
- Develop and adjust individualized care plans in collaboration with the clinical team and physicians
- Educate patients and families, promoting better health outcomes and independence
- Communicate changes in patient condition clearly and efficiently to appropriate team members
Qualifications:
- Current, active RN license
- At least one year of recent experience as a home health RN
- OASIS home health documentation proficiency is required
- Familiarity with HomeCare HomeBase (preferred)
- Valid drivers license and reliable transportation
- Strong organization, time management, and communication skills
