An opportunity is available for a skilled Registered Nurse (RN) specializing in home health care to provide exceptional patient-centered services on a contract basis. This role emphasizes delivering comprehensive nursing care within patients homes, ensuring high-quality outcomes through thorough assessments and individualized care plans. Key Responsibilities:

• Conduct initial and ongoing patient assessments, including vital signs, medication management, and wound care.
• Develop and implement personalized care plans tailored to each patients specific needs.
• Educate patients and their families on disease management, treatment protocols, and preventive measures.
• Collaborate closely with physicians, therapists, and other healthcare professionals to maintain coordinated care.
• Accurately document all patient interactions and care activities in electronic medical records (EMR).
• Monitor patient progress and promptly communicate any changes in condition to the supervising physician.
• Adhere to all applicable state and federal regulations governing home health services.

Qualifications:

• Active Registered Nurse (RN) license.
• Minimum of 3 months home health experience with proficiency using OASIS.
• Experience with HomeCare HomeBase (HCHB) is preferred.
• Strong clinical skills including IV therapy, wound care, and medication administration.
• Proficient in electronic medical record (EMR) systems.
• Excellent communication and organizational skills with the ability to work independently.
• Valid drivers license and reliable transportation for travel to patient homes.
