A rewarding opportunity is available for an experienced Registered Nurse (RN) to provide skilled home health care in West Frankfort, IL. This 13-week full-time contract position offers a chance to apply your clinical expertise and make a meaningful difference in patients lives within their own homes.
Responsibilities:
- Deliver in-home nursing care and skilled interventions
- Conduct thorough patient assessments and ensure timely, accurate OASIS documentation
- Collaborate with physicians and interdisciplinary care teams to develop and implement individualized care plans
- Provide wound care, manage medications, educate patients, and monitor disease progress
- Communicate patient status updates and any changes to the clinical team promptly
Qualifications:
- Active RN license
- Minimum of 1 year recent home health experience
- Proficiency in OASIS documentation is required
- Experience with HomeCare HomeBase is strongly preferred
- Reliable transportation and a valid drivers license
- Ability to manage a structured MondayFriday full-time schedule with daily patient visits
Schedule:
- Full-time, Monday through Friday
- On-call duties likely included
