An exciting opportunity is available for an experienced Registered Nurse specializing in home health. This contract position is perfect for an RN passionate about delivering high-quality care in patients homes. Utilizing your expertise and compassion, you will work closely with patients, their families, and interdisciplinary care teams to create a supportive and healing environment.
Schedule:
- Contract assignment
Key Responsibilities:
- Deliver skilled nursing care to patients in the home setting
- Conduct thorough initial and ongoing patient assessments, including monitoring vital signs, managing medications, and performing wound care
- Design and update individualized care plans tailored to each patients needs
- Educate patients and their caregivers on condition management, treatment, and preventive strategies
- Coordinate with physicians, therapists, and other healthcare professionals for continuity of care
- Accurately document patient interactions and progress in electronic medical records (EMR)
- Observe and report patient status changes to the supervising physician
- Adhere to all applicable regulations governing home health care
Qualifications:
- Current, unrestricted RN license
- At least 3 months of home health experience using OASIS assessment tools
- Home Care Home Base (HCHB) experience strongly preferred
- Clinical expertise in IV therapy, wound care, and medication administration
- Proficiency with EMR systems
- Effective communication and interpersonal skills
- Strong organizational skills and ability to work independently
- Valid drivers license and reliable transportation
