Job Description
An experienced Registered Nurse (RN) with a strong background in home health is needed to provide comprehensive nursing care to patients in their homes on a contract basis. This role emphasizes delivering personalized care, performing detailed patient assessments, and collaborating with healthcare professionals to ensure optimal patient outcomes. Candidates must have experience with OASIS, as it is essential for this position.
Key Responsibilities:
- Provide thorough nursing care to patients in their residences.
- Conduct initial and ongoing patient assessments, including vital signs, medication management, and wound care.
- Develop and implement individualized care plans tailored to patient needs.
- Educate patients and families on disease management, treatment plans, and preventive care strategies.
- Collaborate with physicians, therapists, and other healthcare team members to ensure cohesive care.
- Accurately document patient care activities in electronic medical records (EMR).
- Monitor and report any changes in patient conditions to the supervising physician.
- Ensure compliance with all applicable state and federal home health care regulations.
Qualifications and Desired Experience:
- Active RN license.
- Minimum of 3 months of home health experience utilizing OASIS.
- Experience with Homecare Homebase (HCHB) is highly preferred.
- Strong clinical skills including IV therapy, wound care, and medication management.
- Proficient in electronic medical records (EMR) systems.
- Excellent communication and interpersonal abilities.
- Ability to work independently and manage time efficiently.
- Valid drivers license with reliable transportation.
