Deliver compassionate, expert care as a Registered Nurse specializing in home health services during a 13-week full-time contract in Valparaiso, IN. This position emphasizes clinical excellence and precise OASIS documentation, offering a structured schedule that supports work-life balance in a rewarding healthcare environment.
Schedule:
- Full-time, Monday through Friday
- Potential on-call responsibilities
Key Responsibilities:
- Provide skilled nursing care and perform in-home clinical interventions
- Conduct thorough patient assessments and ensure accurate, timely OASIS documentation
- Collaborate closely with physicians and care teams to develop and implement personalized care plans
- Manage wound care, medication administration, patient education, and chronic disease monitoring
- Communicate effectively with the clinical team regarding patient condition changes
Qualifications:
- Active Registered Nurse (RN) license
- Minimum one year recent experience in home health nursing
- Proficiency with OASIS documentation required
- Experience using HomeCare HomeBase is strongly preferred
- Ability to maintain a structured schedule with daily patient visits
- Reliable transportation and valid drivers license
