A dedicated Registered Nurse (RN) with home health experience is needed for a full-time 13-week contract in Union, OR. This role focuses on delivering skilled nursing care to patients in their homes, ensuring quality clinical interventions and accurate documentation using OASIS standards. The position requires strong clinical skills, attention to detail, and the ability to manage a structured schedule Monday through Friday, with on-call duties likely.
Key Responsibilities:
- Provide in-home nursing care including wound care, medication management, and patient education
- Conduct thorough patient assessments and complete OASIS documentation accurately and promptly
- Collaborate with physicians and care teams to tailor individualized care plans
- Monitor disease progression and communicate updates on patient condition to the clinical team
- Maintain reliable transportation to fulfill daily patient visit requirements
Qualifications:
- Active RN license
- Minimum of 1 year recent home health nursing experience
- Required experience with OASIS documentation
- Proficiency with HomeCare HomeBase software strongly preferred
- Ability to manage a full-time structured schedule with multiple daily visits
- Valid drivers license and reliable transportation essential
