Your next nursing opportunity starts here. We are hiring a committed Registered Nurse (RN) with OASIS expertise for a 13-week full-time home health contract in Tucson, AZ. This assignment offers registered nurses with strong clinical and documentation skills an excellent chance to deliver high-quality care and have a direct impact on patient outcomes in the home setting.
Schedule:
- 13-week assignment, with the potential for extension
- Full-time, Monday through Friday schedule
- On-call responsibilities probable
- Set daily visit expectations
Responsibilities:
- Deliver in-home skilled nursing care and tailored interventions
- Perform comprehensive patient assessments and complete timely, accurate OASIS documentation
- Collaborate with physicians and interdisciplinary teams to develop and implement individualized care plans
- Carry out wound care, medication management, patient education, and chronic disease monitoring
- Communicate any changes in patient condition or care plans promptly to the clinical team
Qualifications:
- Active RN license
- At least 1 year of recent experience as a Home Health RN
- Prior experience with OASIS documentation is required
- Proficiency with HomeCare HomeBase is strongly preferred
- Strong organizational skills and ability to follow a structured, full-time schedule with daily visit requirements
- Reliable transportation and a valid drivers license
