Schedule:
- 13-week contract with potential for extension
- Full-time, Monday through Friday
- On-call responsibilities likely
Qualifications:
- Active RN license required
- At least one year of recent home health RN experience
- Experience with OASIS documentation is mandatory
- Proficiency in HomeCare HomeBase strongly preferred
- Reliable transportation and a valid drivers license
- Comfortable managing a structured daily visit schedule
Primary Responsibilities:
- Deliver skilled nursing care and clinical interventions in patient homes
- Complete comprehensive assessments, with timely and accurate OASIS documentation
- Collaborate with physicians and interdisciplinary care teams to ensure optimal outcomes
- Perform wound care, medication administration, and disease education
- Clearly communicate changes in patient condition to the healthcare team
