Responsibilities:

- Perform patient assessments and ensure accurate, timely completion of OASIS documentation
- Collaborate with physicians and interdisciplinary care teams to develop and execute individualized care plans
- Administer wound care, medication management, patient education, and monitor chronic conditions
- Communicate patient status updates and changes to the clinical team
Qualifications:
- Active Registered Nurse (RN) license
- Minimum one year of recent home health nursing experience
- Proven experience with OASIS documentation
- Proficiency with HomeCare HomeBase software preferred
- Ability to manage a structured full-time schedule, including daily patient visits
- Reliable transportation and a valid drivers license
Job Type:
Contract 13 weeks, with the potential to extend
Schedule:
- Full-time, Monday through Friday
- On-call duties probable
