An opportunity is available for a Registered Nurse (RN) specializing in home health to provide compassionate, patient-centered care on a contract basis. This role requires a professional with a strong background in home health nursing and experience with OASIS to deliver high-quality assessments and individualized care to patients in their homes.
Responsibilities include conducting initial and ongoing patient assessments such as monitoring vital signs, managing medications, and performing wound care. You will develop tailored care plans, educate patients and their families on disease management and preventive measures, and collaborate effectively with a multidisciplinary healthcare team. Accurate and timely documentation in electronic medical records (EMR) is essential, along with monitoring patient conditions and ensuring compliance with all relevant regulations.
Qualifications:
- Active RN license
- Minimum of 3 months home health experience utilizing OASIS
- Experience with HCHB is advantageous
- Clinical proficiency in IV therapy, wound care, and medication management
- Skilled in EMR systems
- Strong communication and interpersonal skills
- Ability to work independently and manage time efficiently
- Valid drivers license and reliable transportation
