Job Description

Qualifications & Desired Experience:

  • Active RN license in good standing
  • Minimum of 1 year recent home health nursing experience
  • Proven experience with OASIS documentation is required
  • Familiarity with HomeCare HomeBase software strongly preferred
  • Ability to follow a full-time, Monday through Friday schedule with on-call availability as needed
  • Reliable transportation and a valid drivers license to attend patient visits

Responsibilities:

  • Deliver skilled nursing care and interventions within patients homes
  • Conduct comprehensive patient assessments and complete OASIS documentation accurately and on schedule
  • Collaborate effectively with physicians and interdisciplinary care teams to tailor individualized care plans
  • Perform wound care, medication management, patient education, and monitor disease progression
  • Communicate timely updates regarding patient status and changes in condition to the clinical team
