A rewarding opportunity awaits for a Registered Nurse (RN) with home health experience to provide quality in-home nursing care on a contract basis in State College, PA. This role emphasizes clinical proficiency, accurate documentation, and the ability to consistently manage a structured schedule. The contract spans 13 weeks, with the possibility to extend based on performance and organizational needs.
Qualifications & Desired Experience:
- Active RN license in good standing
- Minimum of 1 year recent home health nursing experience
- Proven experience with OASIS documentation is required
- Familiarity with HomeCare HomeBase software strongly preferred
- Ability to follow a full-time, Monday through Friday schedule with on-call availability as needed
- Reliable transportation and a valid drivers license to attend patient visits
Responsibilities:
- Deliver skilled nursing care and interventions within patients homes
- Conduct comprehensive patient assessments and complete OASIS documentation accurately and on schedule
- Collaborate effectively with physicians and interdisciplinary care teams to tailor individualized care plans
- Perform wound care, medication management, patient education, and monitor disease progression
- Communicate timely updates regarding patient status and changes in condition to the clinical team
