A dedicated and skilled Registered Nurse with OASIS experience is needed for a 13-week full-time home health contract. This role allows you to apply your clinical expertise, maintain thorough documentation, and independently manage a structured scheduleall while providing crucial care that makes a difference in patients lives.
Schedule:
- 13-week contract, with potential to extend
- Full-time, Monday through Friday
- On-call responsibilities may be required
- Daily visit schedule with clear expectations
Qualifications:
- Active RN license
- At least 1 year of recent experience as a home health RN
- Proven proficiency with OASIS documentation
- Experience with HomeCare HomeBase preferred
- Reliable personal transportation and valid drivers license
- Ability to manage a consistent, full-time patient visit schedule
Key Responsibilities:
- Deliver skilled nursing care in patient homes, ensuring safety and quality standards
- Complete comprehensive assessments and submit accurate, timely OASIS documentation
- Collaborate with physicians and members of the healthcare team on individualized care plans
- Perform wound care, medication management, disease monitoring, and patient/caregiver education
- Communicate promptly and professionally regarding changes in patient condition
