A hospice registered nurse (RN) position is available in Sedona, AZ, on a contract basis. This role offers the opportunity to provide compassionate, end-of-life care to patients and support their families throughout this important journey.
Key Responsibilities:
- Deliver skilled nursing care and symptom management to hospice patients.
- Conduct thorough clinical assessments and develop individualized care plans.
- Collaborate with interdisciplinary teams to coordinate patient care.
- Offer emotional support and education to patients and their families.
- Maintain accurate and timely documentation in accordance with hospice guidelines.
Qualifications:
- Valid Registered Nurse (RN) license.
- Minimum of 2 years experience in hospice or palliative care nursing.
- Proficiency with Home Care Homebase (HCHB) software preferred.
- Strong clinical assessment skills and effective communicator.
- Compassionate and empathetic patient-centered approach.
- Reliable transportation and a valid drivers license.
