Job Description

A hospice registered nurse (RN) position is available in Sedona, AZ, on a contract basis. This role offers the opportunity to provide compassionate, end-of-life care to patients and support their families throughout this important journey.

Key Responsibilities:

  • Deliver skilled nursing care and symptom management to hospice patients.
  • Conduct thorough clinical assessments and develop individualized care plans.
  • Collaborate with interdisciplinary teams to coordinate patient care.
  • Offer emotional support and education to patients and their families.
  • Maintain accurate and timely documentation in accordance with hospice guidelines.

Qualifications:

  • Valid Registered Nurse (RN) license.
  • Minimum of 2 years experience in hospice or palliative care nursing.
  • Proficiency with Home Care Homebase (HCHB) software preferred.
  • Strong clinical assessment skills and effective communicator.
  • Compassionate and empathetic patient-centered approach.
  • Reliable transportation and a valid drivers license.
