An opportunity awaits for an experienced Registered Nurse skilled in home health care to deliver critical support on a contract basis. This role enables you to make a substantial impact by providing attentive, compassionate care to patients in the comfort of their homes, and draws on your expertise in OASIS and electronic medical records.
Schedule/Hours
- Contract assignment with potential for extension
Qualifications
- Active RN license
- At least 3 months of home health experience using OASIS
- HCHB experience highly valued
- Strong clinical skills: IV therapy, wound care, medication management
- Proficiency with EMR systems
- Outstanding communication and interpersonal abilities
- Self-motivated with strong time management
- Valid drivers license and reliable transportation
Responsibilities
- Deliver comprehensive nursing care in patients homes
- Perform initial and ongoing patient assessments, including vitals and wound care
- Create and implement individualized care plans
- Educate patients and families on disease management, treatments, and preventive care
- Collaborate with interdisciplinary healthcare teams to coordinate care
- Accurately document patient interactions and care in EMR systems
- Monitor, evaluate, and report patient condition changes to physicians
- Adhere to state and federal home health care regulations
