A dedicated Registered Nurse (RN) with home health and OASIS experience is needed for a 13-week full-time contract in Scranton, PA. This role offers a structured schedule focused on providing in-home nursing care, conducting thorough patient assessments, and completing accurate OASIS documentation. The position is ideal for an RN who thrives in managing daily patient visits while collaborating closely with physicians and care teams to deliver personalized care.
Responsibilities:
- Provide skilled nursing care and interventions in patients homes
- Conduct comprehensive patient assessments and complete OASIS documentation on time
- Collaborate with physicians and other healthcare professionals to develop and implement individualized care plans
- Perform wound care, medication management, patient education, and monitor disease progression
- Communicate patient updates and condition changes promptly to the clinical team
Qualifications and Desired Experience:
- Active Registered Nurse (RN) license
- Minimum of 1 year recent home health RN experience is required
- Proficient in OASIS documentation; experience with HomeCare HomeBase is strongly preferred
- Ability to manage a structured full-time schedule with daily patient visits
- Reliable transportation and a valid drivers license
Contract Details:
- 13-week full-time contract with potential extension
- Monday through Friday schedule with possible on-call duties
