This contract position offers an opportunity for an experienced Registered Nurse (RN) with a home health background to provide compassionate, patient-centered care in a home setting. A strong emphasis on OASIS documentation and assessment is essential for this role. Candidates with solid clinical skills in IV therapy, wound care, and medication management will thrive in this position.
Key Responsibilities:
- Deliver comprehensive nursing care to patients within their homes.
- Conduct initial and ongoing patient assessments, including vital signs and medication oversight.
- Develop and execute individualized care plans tailored to patient needs.
- Educate patients and families on disease management, treatment protocols, and preventive strategies.
- Collaborate closely with physicians, therapists, and healthcare providers to ensure integrated care.
- Accurately document all patient care activities in electronic medical records (EMR).
- Monitor changes in patient conditions and promptly report to supervising physicians.
- Maintain compliance with all applicable state and federal home health care regulations.
Qualifications:
- Active Registered Nurse (RN) license.
- Minimum of three months of home health experience utilizing OASIS.
- Experience with Home Care Home Base (HCHB) software is highly desirable.
- Proficient clinical skills including IV therapy, wound care, and medication administration.
- Comfortable with electronic medical records (EMR) systems.
- Strong communication and interpersonal abilities.
- Ability to work independently with effective time management.
- Valid drivers license and reliable transportation.
