A full-time home health nursing opportunity is available for a Registered Nurse (RN) with strong clinical and documentation skills, including OASIS experience. This 13-week contract position offers a structured schedule, working Monday through Friday, with the possibility of on-call duties. It is designed for a nurse comfortable managing daily patient visits while delivering high-quality, compassionate care in patients homes.
Responsibilities include providing skilled nursing interventions, performing patient assessments, and completing accurate OASIS documentation in a timely manner. Collaboration with physicians and care teams to create and implement individualized care plans is essential. Key nursing duties such as wound care, medication management, patient education, and disease monitoring will be regularly performed. Effective communication regarding patient status and any changes is crucial to support clinical decision-making.
Preferred qualifications for this role:
- Active Registered Nurse license
- At least one year of recent home health RN experience
- Proven experience with OASIS documentation
- Familiarity with HomeCare HomeBase software is strongly preferred
- Ability to manage a full-time schedule with daily home visits
- Reliable transportation and a valid drivers license
