We are seeking an experienced Registered Nurse with strong OASIS skills for a dynamic 13-week full-time assignment. Bring your clinical expertise, detail-oriented documentation, and a proactive approach to patient care to a supportive care team.
Schedule & Hours:
- 13-week full-time contract, Monday through Friday
- On-call responsibilities may be assigned
- Daily visit expectations with a structured caseload
Key Responsibilities:
- Deliver in-home nursing care, including skilled interventions and evaluations
- Complete patient assessments and OASIS documentation accurately and promptly
- Work closely with physicians and care teams to tailor individualized care plans
- Perform wound care, medication administration, disease monitoring, and provide patient education
- Maintain clear communication regarding patient conditions and updates with the clinical team
Required Qualifications:
- Active RN license in good standing
- At least one year of recent RN home health experience
- OASIS proficiency is required
- Familiarity with HomeCare HomeBase is strongly preferred
- Strong organizational and time-management skills to handle a structured daily schedule
- Reliable transportation and a valid drivers license
