Job Description
A dedicated Registered Nurse (RN) with home health experience is needed for a 13-week full-time contract in Rocky Mount, VA. This role offers the opportunity to deliver skilled nursing care in patients homes, focusing on clinical excellence and compassionate service within a structured schedule.
Key Responsibilities:
- Provide comprehensive in-home nursing care and skilled interventions
- Conduct thorough patient assessments and complete OASIS documentation accurately and on time
- Collaborate with physicians and care teams to develop and implement individualized care plans
- Perform wound care, medication management, patient education, and monitor disease progression
- Communicate patient status updates and changes effectively to the clinical team
Qualifications / Desired Experience:
- Active Registered Nurse (RN) license
- Minimum of one year recent experience in home health nursing
- Proven proficiency with OASIS documentation (required)
- Experience with HomeCare HomeBase is strongly preferred
- Ability to manage a structured full-time schedule with daily patient visits
- Reliable transportation and a valid drivers license
Schedule/Hours:
- Full-time, Monday through Friday
- On-call duties probable
