Were seeking a Registered Nurse with strong OASIS experience to deliver high-quality in-home care through a 13-week full-time contract. Schedule:
- Full-time, Monday through Friday
- Daily visit structure (on-call responsibilities likely)
Key Qualifications:
- Active RN license
- Minimum of 1 year of recent RN experience in home health
- OASIS documentation proficiency
- Experience using HomeCare HomeBase (strongly preferred)
- Reliable transportation and a valid drivers license
- Demonstrated ability to manage a full caseload with effective time management
Primary Responsibilities:
- Deliver skilled nursing care, including assessments, medication management, and wound care, directly in patients homes
- Complete accurate, timely OASIS documentation and care plans
- Partner with physicians and interdisciplinary teams to create individualized patient strategies
- Provide patient and family education on disease process and self-care techniques
- Monitor and report any significant changes in patient conditions to the wider clinical team
