A Registered Nurse (RN) with home health experience is needed for a 13-week contract opportunity in Richland, Oregon. This role allows for full-time clinical practice focused on providing skilled nursing care in patients homes, with an emphasis on OASIS documentation and personalized patient management. Ideal candidates demonstrate strong clinical skills and are comfortable managing a structured schedule with daily patient visits.

Responsibilities:
- Deliver in-home nursing care and skilled interventions aligned with patient needs
- Perform thorough patient assessments and ensure accurate, timely OASIS documentation
- Collaborate closely with physicians and interdisciplinary care teams to implement individualized care plans
- Manage wound care, medication administration, patient education, and chronic disease monitoring
- Communicate patient status updates and changes to the clinical team promptly
Qualifications / Desired Experience:
- Active Registered Nurse license
- Minimum one year of recent home health RN experience with OASIS proficiency
- Experience with HomeCare HomeBase highly preferred
- Ability to manage a structured full-time schedule, including daily patient visits
- Reliable transportation and a valid drivers license
Contract Details & Benefits:
