A dynamic contract role is open for an experienced Registered Nurse specializing in home health care. Ideal candidates are skilled in OASIS documentation, thrive on patient-centered care, and are organized in managing a full visit schedule. This 13-week assignment offers the chance to make an immediate difference in patients lives while working with a dedicated clinical team.
Schedule:
- Full-time, Monday through Friday
- On-call responsibilities may be required
- Structured daily visit expectations
Key Responsibilities:
- Deliver in-home nursing care and skilled clinical interventions to diverse patients
- Complete comprehensive patient assessments and ensure timely, accurate OASIS documentation
- Partner closely with physicians and multidisciplinary care teams to provide individualized treatment plans
- Perform wound care, manage medications, deliver patient and family education, and oversee disease process monitoring
- Communicate updates or changes in patient conditions promptly to the clinical leadership team
Qualifications:
- Active RN license
- Minimum of 1 year recent home health RN experience
- OASIS documentation experience required
- Proficiency with HomeCare HomeBase (strongly preferred)
- Demonstrated ability to manage a structured, full-time patient visit schedule
- Valid drivers license and reliable transportation
