We have an immediate opening for a Registered Nurse specializing in home health care, offering you the chance to deliver essential services and shape meaningful patient experiences during a 13-week contract. This full-time role is ideal for an RN confident in clinical delivery, documentation, and managing a robust schedule.
Schedule/Hours:
- Full-time, Monday through Friday
- On-call rotation may be required
- 13-week contract with the option to extend
Qualifications:
- Current, active RN license
- At least 1 year of recent home health RN experience
- Proficient in OASIS documentation (required)
- Strong working knowledge of HomeCare HomeBase (strongly preferred)
- Reliable transportation and valid drivers license
- Demonstrated organizational skills and ability to manage multiple daily visits
Responsibilities:
- Deliver skilled in-home nursing care, including wound care, medication management, patient education, and disease monitoring
- Accurately complete patient assessments and timely OASIS documentation
- Collaborate effectively with physicians and interdisciplinary care teams to develop custom care plans
- Provide regular updates regarding patient condition to the clinical team
- Maintain compliance with home health regulatory standards and best practices
