We are seeking an experienced Registered Nurse for an impactful role providing in-home care for patients in the Portland, OR area. This full-time contract position offers the chance to make a difference in patients lives while working within a structured, supportive environment.
Schedule:
- 13-week assignment, with potential to extend
- Monday through Friday, full-time hours
- On-call responsibilities may be required
Key Responsibilities:
- Deliver skilled nursing care and clinical interventions in patients homes
- Complete thorough patient assessments and submit accurate, timely OASIS documentation
- Collaborate closely with physicians and interdisciplinary teams to customize care plans
- Perform wound care, medication administration, patient/family education, and disease management
- Maintain clear and consistent communication regarding patient updates and condition changes
Qualifications:
- Active RN license
- At least 1 year of recent professional experience as a home health RN
- Solid understanding and hands-on experience with OASIS documentation
- Proficiency with HomeCare HomeBase (strongly preferred)
- Proven ability to manage a structured, busy daily patient schedule
- Reliable transportation and current drivers license
