A skilled Registered Nurse (RN) with home health experience is needed to provide compassionate, high-quality care to patients in their own homes. This contract position offers the opportunity to make a meaningful impact through personalized nursing services while working independently within a supportive team framework.
Key Responsibilities:
- Deliver thorough nursing care including initial and ongoing assessments, vital sign monitoring, medication management, and wound care.
- Develop and execute individualized care plans tailored to each patients specific needs.
- Educate patients and their families on managing diseases, treatment protocols, and preventive health strategies.
- Collaborate effectively with physicians, therapists, and other healthcare professionals to coordinate comprehensive care.
- Accurately document all patient care activities in electronic medical records (EMR) promptly.
- Monitor changes in patient conditions and communicate updates to supervising healthcare providers.
- Ensure compliance with state and federal home health regulations.
Qualifications:
- Active Registered Nurse (RN) license.
- Minimum of 3 months experience in home health utilizing OASIS.
- Experience with Home Care Home Base (HCHB) software is a significant advantage.
- Proficient clinical skills including IV therapy, wound care, and medication administration.
- Strong familiarity with electronic medical records (EMR) systems.
- Excellent communication and interpersonal abilities.
- Ability to work independently and manage time efficiently.
- Valid drivers license and dependable transportation.
