Job Description
A full-time Registered Nurse (RN) with experience in home health and OASIS documentation is needed for a 13-week contract assignment based in Pittston, PA. This role requires strong clinical skills, excellent documentation accuracy, and the ability to follow a structured schedule while delivering compassionate in-home nursing care.
Key Responsibilities:
- Provide skilled nursing interventions and in-home care
- Conduct patient assessments and complete OASIS documentation accurately and timely
- Collaborate with physicians and care teams to implement individualized care plans
- Perform wound care, medication management, patient education, and monitor disease progression
- Communicate patient status updates and changes to the clinical team
Qualifications:
- Active RN license
- At least 1 year of recent home health RN experience
- Proven experience with OASIS documentation
- Proficiency with HomeCare HomeBase is strongly preferred
- Ability to manage a full-time, structured schedule with daily patient visits
- Reliable transportation and a valid drivers license
Contract Details:
- 13-week assignment with potential for extension
- Full-time schedule, Monday through Friday, with possible on-call requirements
