Key Responsibilities:
- Deliver skilled nursing care and interventions in patient homes
- Conduct thorough patient assessments and complete OASIS documentation accurately and punctually
- Collaborate effectively with physicians and multidisciplinary care teams to implement individualized care plans
- Manage wound care, medication administration, patient education, and monitor chronic conditions
- Communicate significant updates and changes in patient conditions to the clinical team
Qualifications & Desired Experience:
- Active Registered Nurse licensure
- Minimum of 1 year recent experience in home health nursing
- Proven experience with OASIS documentation is required
- Proficiency with HomeCare HomeBase is highly preferred
- Ability to manage a structured, full-time schedule with daily patient visits
- Reliable transportation and valid drivers license
Schedule:
- Full-time, Monday through Friday
- On-call availability likely required
