Key Responsibilities:
- Deliver compassionate in-home nursing care and skilled clinical interventions
- Complete thorough patient assessments and timely, accurate OASIS documentation
- Collaborate with physicians and multidisciplinary care teams to implement individualized care plans
- Provide wound care, medication management, disease process education, and health monitoring
- Communicate patient status changes and pertinent updates to the clinical team
Preferred Qualifications:
- Active and unencumbered RN license
- Minimum 1 year of recent home health RN experience
- Strong proficiency with OASIS documentation
- HomeCare HomeBase experience highly preferred
- Reliable transportation and valid drivers license
- Demonstrated ability to manage a full-time schedule with structured patient visits
