Your next nursing opportunity starts here. We are searching for an experienced Registered Nurse (RN) with a strong background in home health and OASIS documentation for a 13-week full-time contract assignment. This role is an excellent chance to leverage your clinical expertise in a setting where patient relationships and outcomes truly matter.
Schedule & Assignment Details:
- 13-week contract with the potential to extend
- Full-time commitment: Monday through Friday
- Daily visit targets with the probability of on-call duty
Key Responsibilities:
- Deliver in-home skilled nursing care, including assessments and interventions
- Complete thorough and timely OASIS documentation for each patient
- Create and execute individualized care plans in conjunction with physicians and interdisciplinary teams
- Oversee wound care, administer medications, monitor chronic conditions, and educate patients/families
- Communicate changes in patient status clearly and promptly to the clinical team
Qualifications & Experience:
- Unrestricted RN license
- At least 1 year of recent RN experience in home health
- Proficient with OASIS documentation (required)
- Experience using HomeCare HomeBase is highly valued
- Reliable transportation and a valid driver's license
- Comfortable managing a structured, full-time visit schedule
