Schedule:
- 13-week contract with potential for extension
- Full-time, Monday through Friday
- On-call participation may be required
- Structured daily visit expectations
Key Qualifications:
- Active RN license
- Minimum of 1 year recent experience in home health as an RN
- Proficiency in OASIS documentation (required)
- Experience with HomeCare HomeBase (strongly preferred)
- Reliable transportation and valid driver's license
- Ability to manage a full-time structured schedule with daily visits
Responsibilities:
- Deliver in-home nursing care and skilled interventions
- Complete comprehensive patient assessments and timely OASIS documentation
- Collaborate with physicians and interdisciplinary care teams on tailored care plans
- Provide wound care, medication management, patient education, and disease monitoring
- Regularly communicate patient updates and changes in condition
