Job Description

An opportunity is available for a Registered Nurse (RN) with expertise in home health care to provide compassionate and comprehensive nursing services in patients' homes on a contract basis. This role is ideal for an experienced nurse passionate about delivering personalized care and improving patient outcomes in the Pearl, MS area.

Key Responsibilities:

  • Conduct thorough initial and ongoing patient assessments, including monitoring vital signs, managing medications, and providing wound care.
  • Develop and implement individualized care plans tailored to each patients specific needs.
  • Educate patients and their families on disease management, treatment protocols, and preventive care strategies.
  • Collaborate closely with physicians, therapists, and other healthcare professionals to ensure coordinated and effective care.
  • Accurately document all patient care activities using electronic medical records (EMR).
  • Monitor patient conditions vigilantly and report any changes promptly to the supervising physician.
  • Adhere to all applicable state and federal regulations governing home health services.

Qualifications:

  • Active Registered Nurse (RN) license.
  • Minimum of 3 months of home health experience utilizing OASIS assessment tools.
  • Experience with Homecare Homebase (HCHB) software is a significant advantage.
  • Strong clinical skills including IV therapy, wound care, and medication management.
  • Proficiency with electronic medical records (EMR) systems.
  • Excellent communication and interpersonal skills.
  • Ability to work independently with effective time management.
  • Valid drivers license and reliable transportation.
