An opportunity is available for a Registered Nurse (RN) with expertise in home health care to provide compassionate and comprehensive nursing services in patients' homes on a contract basis. This role is ideal for an experienced nurse passionate about delivering personalized care and improving patient outcomes in the Pearl, MS area.
Key Responsibilities:
- Conduct thorough initial and ongoing patient assessments, including monitoring vital signs, managing medications, and providing wound care.
- Develop and implement individualized care plans tailored to each patients specific needs.
- Educate patients and their families on disease management, treatment protocols, and preventive care strategies.
- Collaborate closely with physicians, therapists, and other healthcare professionals to ensure coordinated and effective care.
- Accurately document all patient care activities using electronic medical records (EMR).
- Monitor patient conditions vigilantly and report any changes promptly to the supervising physician.
- Adhere to all applicable state and federal regulations governing home health services.
Qualifications:
- Active Registered Nurse (RN) license.
- Minimum of 3 months of home health experience utilizing OASIS assessment tools.
- Experience with Homecare Homebase (HCHB) software is a significant advantage.
- Strong clinical skills including IV therapy, wound care, and medication management.
- Proficiency with electronic medical records (EMR) systems.
- Excellent communication and interpersonal skills.
- Ability to work independently with effective time management.
- Valid drivers license and reliable transportation.
